All entries to the Six Minute Cinema Digital Filmmaking Festival must be accompanied by an Official Entry Form which has been completely and correctly filled out.

To open a copy of the PDF Entry Form in your web browser single-click on the link below. To copy it to your Windows hard drive right-click on the link and choose "Save Target As." On a Mac click the link and hold until a menu appears, then choose "Download Link to Disk."

Print out as many copies as you need for your entries. Remember that a single production can be entered in multiple categories with a single Entry Form and check, but multiple VHS copies must be provided - one for each category entered.

Separate productions must have individually completed Entry Forms.
